Actual Accomplishments: Records what the employee finished during the rating period.
Rating Scale: Uses a 1-to-5 numerical rating scale evaluated by the employee (self-rating) and validated by the immediate supervisor.
Signatures: Requires sign-offs from the employee, immediate supervisor, and head of office.
